Before Surgery

Patient will receive paperwork by their surgeon's office or by our center which will need to be completed upon arrival to our center. You are encouraged to download this information and bring it completed on the day of your surgery. This information will also include pre operative instructions for patients receiving IVB (Intravenous Block), MAC, or General Anesthesia.

Patients receiving General Anesthesia or Monitored Anesthesia Care will receive a telephone call the day before surgery by an anesthesiologist, who will assess the patient, ask for a brief medical history, gives instructions and personally answers questions the patient may have. The information that you provide will help in determining your physical status, which enables us to provide the best care possible.

On The Day of Surgery

When you arrive at the De La Vina Surgicenter, a receptionist will greet you and ask you to complete the necessary paperwork. You will also be asked to provide your insurance card so we may scan it into our system. Patients are admitted to private care areas by one of our nurses. The anesthesiologist will meet with the patient before surgery to discuss the anesthetic plan.

Following Surgery

Once surgery is complete, patients will go to the Post Anesthesia Care Unit (PACU) where monitoring is conducted by skilled nurses.

When ready, patients will then go to Step-Down, where family may join you. Prior to being discharged, each patient is given instructions on proper follow-up care. A nurse from our center will telephone you the next day to check on your progress.

Hours of Operation

The hours of operation are Monday through Friday, 6:00 a.m. until 5:00 p.m.


Patients are registered at De La Vina Surgicenter on the day they receive their services by one of our Patient Intake Coordinators. Patients are asked to arrive one hour prior to their scheduled surgery time. Please feel free to contact our center if you should have any questions about your arrival time.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
